About this notice

This notice explains how StateWorkersComp.com handles information when you visit our website, use our calculator, or contact us. Last updated November 3, 2026.

Information you provide

The calculator collects your business name, contact name, email address, phone number, business address, classification code, business entity type, full-time and part-time employee counts, and annual payroll when you submit a request. We use these details and any rate you enter to calculate an estimate. We retain your submitted details for insurance follow-up, including the rate used and estimate when one is available. If we have no rate for your classification, we still retain your request so an agent can price it. We also retain the page where you arrived, the referring website when available, campaign tags or advertising click identifiers included in your link, the page where you submitted, the date of your request, and records of the notice shown to you. We do not collect unfinished forms as submitted requests.

Contact inquiries

When you submit our contact form, we save your name, email address, selected topic, message, source information, and submission time. We share your inquiry with our response team at glgins.com so they can address it, including coverage questions, privacy requests, and security reports. Cloudflare stores these records and Resend processes email notifications. Please do not send passwords, medical records, or other sensitive documents through this form.

Saving your progress

Your browser keeps a temporary draft of your entries and current step so you can refresh the page without starting over. This includes any rate you enter. We also remember how you arrived during that browsing session to understand the source of submitted requests. The draft normally clears when the tab’s browsing session ends, although your browser’s restore-session feature may bring it back. Drafts are not sent for agent follow-up until you submit the calculator.

How we use and share requests

We use submitted information to provide your estimate and help arrange insurance-agent follow-up. We share your business and contact details, location, classification, payroll, the rate used, estimate, and source information with the team receiving requests at glgins.com for that purpose. Cloudflare provides website hosting and data storage, and Resend processes email notifications on our behalf. We also retain notification records to track requests and avoid duplicate messages. Submitting a request does not guarantee a response or an insurance offer.

ZIP-code lookup

When you enter a five-digit ZIP code, Zippopotam.us receives that ZIP code and connection information, including your IP address, to suggest your city and state. Your other calculator entries are not included in that lookup. The provider’s privacy practices apply to its handling of this information.

Website visits

Our hosting provider processes technical information such as your IP address, browser information, and requested pages to deliver and protect the website. We use Google Ads (through the Google tag) and Meta Pixel to see which ads bring visits and saved requests, and to show our ads to people who visited the site. Unless you opt out, these companies receive the pages you visit, the site that sent you, your IP address, browser and device details, cookie or ad IDs, and calculator events: when you finish step 1, when you finish step 2, and when your request is saved. When your request is saved, Google also receives your email address and phone number in hashed form (scrambled before it leaves your browser) so it can match the request to an ad click. We do not send your business name, address, payroll, class code, or estimate to Google or Meta. Under some state privacy laws, this ad measurement may count as a sale or sharing of personal information. Source information from your visit is saved with your request to understand which pages and campaigns bring inquiries. The calculator draft does not use cookies or permanent browser storage.

Advertising and privacy choices

Use the Do Not Sell or Share My Personal Information control on this page or in the footer to opt out. When you opt out, this browser does not load the Google tag or Meta Pixel, and we send no page, calculator, or request events to either company. We treat a Global Privacy Control signal from your browser the same way, even if you never use our control. Our preference cookie remembers a manual opt-out for one year on this browser; clearing cookies or using another browser or device may require a new choice. An opt-out does not stop the essential processing of an estimate or contact request, or the insurance follow-up you request. You may ask about access, correction, deletion, or other rights available under applicable law through our contact form. We do not require an account to opt out of advertising.

Retention and access

Submitted requests and follow-up records are retained until deleted; there is no fixed retention period. Access is limited to people and service providers handling the website and your request. We do not publish your contact details or submitted requests on the website.
